Complaints and Investigations

The Ohio Revised Code authorizes the Board to investigate compliance with the laws and rules governing the practice and to investigate alleged grounds for discipline of a license or refusal to issue a license. The following documents include an explaination of the complaint and investigation process and the actual form needed to file a complaint against a licensee.

Formal Board Actions

The following documents list the formal board actions taken by the Board during the given month. At the end of the calendar year, the monthly reports will be consolidated into a single document listing all actions taken by the Board in the year. To view the public records related to the formal board action, use the License Lookup/Verification link in the Information menu.
