The following documents contain the geographic distribution of active licensees by county of residence.
To obtain a licensure mailing list for Occupational Therapist, Occupational Therapy Assistants, Physical Therapist, Physical Therapist Assistants, and/or Athletic Trainers, submit an email request to board@otptat.ohio.gov and indicate the professional listing(s) you would like to obtain. Please specify in this email request if you would like the listing to include the licensees' phone number and/or email address. The mailing list includes the following fields: licensee name; license number, complete mailing address; county; original license issue date; license expiration date; and practice setting. The licensees' phone number and email address can be provided upon request. The mailing lists are provided in Excel format and there is no charge to receive the list as an Excel attachment via email. Please allow approximately 2-3 business days to process your request.
