Online Applications

Individuals can submit an online examination and endorsement application through the Ohio e-License Center as an alternative to downloading and mailing the paper application. A link to the Online Initial License Application site is located on the Information menu on the left-hand column of this page. After applying online, applicants are required to submit the Certification of Applicant for Online Applications document, in addition to any other supporting materials required as part of the application. 

Downloadable Paper Applications

• Examination Application - This application is for individuals seeking licensure in Ohio who do not hold an athletic training license in another U.S. state or territory.

• Endorsement Application - This application is for individuals seeking licensure in Ohio who hold a current, valid license to practice athletic training in another U.S. state or territory.

• Reinstatement Application - This application is for individuals who previously held a license to practice athletic training in Ohio at any time.

• Athletic Training Event Approval Application - The Ohio Athletic Training Practice Act requires the organizer of an event at which non-Ohio licensed athletic trainers will be performing athletic training services in Ohio to have the event approved by the Athletic Trainers Section no later than 30 days before the date of the event.
